Service Description

Provides long term “home based” support for individuals who have secured housing. Home Visitors deliver direct supports in the home, ensuring they have access to resources and supports necessary to maintain their housing. Support is provided in matters relating to housing, physical and mental health, addictions, advocacy, justice, income support, and employment. Services include:

  • Initiating and maintaining regular and long term intensive in home support,
  • Establishing a voluntary and trusting relationship by providing regular contact through home visits,
  • Crisis management and support,
  • Advocacy,
  • Knowledge in life skills, budgeting, home maintenance and upkeep, renter’s rights and responsibilities, food security, and rent security, and
  • Assistance in accessing resources and making referrals.

Eligibility

Anyone who identifies as homeless and is living with mental health and addictions concerns.
