Service Description

Provides support and follow up once families with a newborn baby are discharged from the hospital.

  • Telephone follow-up, clinic appointments, or home visits are provided by Registered Nurses and International Board Certified Lactation Consultants.

Healthy and Home will:

  • Contact you by telephone or home visit within one-two days after a hospital discharge
  • Assess the Mom’s recovery from the birth
  • Assess the baby’s health and weight
  • Assist with breastfeeding
  • Provide health information, answer questions, and make referrals as needed.

Eligibility

Families with children birth - 2 weeks.
