Service Description
Provides support and follow up once families with a newborn baby are discharged from the hospital.
- Telephone follow-up, clinic appointments, or home visits are provided by Registered Nurses and International Board Certified Lactation Consultants.
Healthy and Home will:
- Contact you by telephone or home visit within one-two days after a hospital discharge
- Assess the Mom’s recovery from the birth
- Assess the baby’s health and weight
- Assist with breastfeeding
- Provide health information, answer questions, and make referrals as needed.
Eligibility
Families with children birth - 2 weeks.
